Case Management Services for Adults Ages: 18 and older

Case Management Services partners community support specialists with the individual to assess needs, obtain services, treatments, support and prevent and manage crisis. The goal of Case Management is recovery and self-management; teaching individuals the skills they need to be stronger and healthier; and serving as advocates for the individual's needs. Community support specialists link, refer, advocate and skill build for individuals in the communities where they live.
Eligibility: Age 18 and older, with a mental health diagnosis, and where assessment supports need for case management services.
