A System of Care provides wraparound services through community involvement and resources with the goal of keeping children and adolescents successful in their home & community.The System of Care team uses a strengths-based approach and family-driven care to develop the wraparound family plans.
Eligibility: A child in the family must be under the age of 19 years, and must meet one of the following criteria: 1) Be at risk of being moved out of the family home for placement, or is currently placed outside of the home. 2) Be involved with two or more of the following services: school, Division of Child Services, Probation, or mental health counseling/services. Includes children who are seriously emotionally disturbed and adolescents receiving services from multiple agencies.
