Intervention Services for Addiction Disorders Ages: All ages
State: TN
Working with family, friends or an employer, an Interventionist uses a team approach to confront an individual about the impact of their addiction. Through the intervention, the individual is encouraged to seek treatment. If the person agrees to treatment, the Interventionist provides the individual with professional and confidential advice in addressing addiction-related issues, assistance with entering the most appropriate treatment program, as well as transportation to treatment and case coordination during and following completion of treatment.

Weems Academy Ages: 6-22

Weems Academy is for youth who have emotional and behavioral problems that prevent them from functioning in a regular school setting. Public school coordinators refer students to Weems Academy where they receive counseling and therapy while maintaining their academic studies. The goal is to return the youth to their community school.

Building Strong Families Ages: 18 and younger
State: TN
Building Strong Families is a program for children, birth to age 17, who are at imminent risk (within 24 hours) of being placed outside the home because of their parent's or caretaker's drug use. This program provides intensive family preservation services. The program provides crisis intervention, intensive in-home therapy for all members of the family, parenting and life skills training and referral to other needed services, such as substance abuse treatment. It is designed to keep children and families together. This is a free program provided for by a grant.

Therapeutic Preschool Ages: 30 months to 5 years

For over 20 years, Centerstone’s innovative Therapeutic Preschool program has provided comprehensive evaluation and intensive treatment for children, ages 30 months to five years-old. This program consists of individual therapy, family therapy and case management services. Some of the services are provided at our clinic and many are provided in the child's preschool or daycare classroom. Our preschool staff work with the family and the classroom teacher so that the child can be successful in their preschool/daycare setting.

Housing specialists administer funding for independent living assistance, as well as focus on determining housing needs of homeless or displaced individuals. Specialists also link individuals with suitable housing based on their needs.

For youth in need of support and adult role models in the foster care system. These adults share their personal time doing positive activities with youth on weekends. Mentors must be at least 21 years of age with full-coverage insurance and a driver's license.

Regional Intervention Program (RIP) Ages: Children 6 and younger and their parents

RIP is a nationally recognized, parent-implemented program designed to help parents deal with the behavior and development of preschool children. Experienced parents who have completed the program, along with professional staff, successfully provide training and support to newly enrolled families. Services to families through RIP are free due to a Tennessee Department of Mental Health grant, United Way and other local contributions.

Get REAL (Get REAL, Get Tested!) Ages: 12-17

This prevention program provides evidence-based services designed to reduce the onset of substance abuse and transmission of HIV/AIDS with youth ages 12-17. Prevention Specialists facilitate the evidence-based curricula Keepin' it REAL and Be Proud! in weekly small group session and the Outreach Specialist provides community-based edducation and outreach. This program offers free HIV testing to participants. Get REAL is funded by a grant from the Substance Abuse and Mental Health Services Administration/Center for Substance Abuse Prevention.

Project BASIC (Better Attitudes and Skills in Children) Ages: Kindergarten through third grade students

This social skills enhancement program provides educational activities for children, grades K-3, to help students build self-esteem and learn good decision-making skills. The foundation laid will help students to make good choices now and throughout the years to come. Prevention Specialists enter the classroom weekly and lead student activities. Co-Occurring Disorders Treatment Ages: 18 and older
State: TN
Co-Occurring Disorders Treatment is for individuals who have both a substance use disorder and a mental illness. This program provides therapy, intensive outpatient counseling, case management and psychiatric services for both disorders, and may include rapid HIV testing for individuals and their partners, limited assistance with transportation to treatment and referral to primary care when needed. Individuals with co-occurring disorders have a better chance of recovery from both disorders when they receive services that combine mental health and substance abuse treatment. Treatment may be provided free of charge due to a federal grant.

Adult supported housing services assist individuals in trasition from psychiatric hospitalization or unstable living situations to stable and more independent living situations.
Eligibility: An adult who is in need of transitional residential supervision. This can either be utilized as a step-down transition from the hospital to community living or as a diversion.

This is a comprehensive substance abuse prevention program targeting youth (ages 10-18) who are at high risk for substance use/abuse. Be Sharp uses Keepin' it REAL, an evidence-based curriculum, along with service learning activities. All 12 group sessions are provided in community-based settings to enhance individual knowledge and skills necessary to develop healthy communication/negotiation skills, anti-drug attitudes and avoidance of high-risk behaviors. Child and Youth Residential Treatment Services - Youth Group Homes Ages: 11-18
State: TN
These are highly structured programs addressing problems or behaviors keeping youth from being successful in the community. Residency lasts two to three months. While living in the home, youth attend public schools, participate in community recreation and learn independent living skills such as food preparation, banking, laundry and shopping.
The Lodge is a 6-bed residential facility (Level II) for girls ages 11 to 18 in Montgomery County. The average stay is 1 to 3 months. Services focus on rapid stabilization and quick transition back to home and community. Individuals are also linked to mental health treatment and other supports to help prevent the occurrence of a future crisis.

Criminal Justice Liaisons provide continuity of care for individuals who have been incarcerated or are at risk of incarceration in designated counties. Case Management Services for Children and Youth Ages: 17 and under

Case Management Services is a voluntary in-home intervention and advocacy service. It is designed, whenever possible, to keep children and families together. The goal is to teach the child and families the skills they need to be stronger and healthier, and to be able to advocate for the child's needs. Case Managers link, refer, advocate and skill-build for families in the communities where they live.
Eligibility: Must be under 18 with a mental health diagnosis or a severe emotional disturbance. Has a history of out of home placements or difficulty in the home environment. Has difficulty adjusting to their natural environment - school, home, and/or community.

Coaching4Teens—Life Coaching for Teens Ages: High School Students

Coaching4Teens offers personal coaching for high school students in the Nashville area. Coaching is a new concept that is sweeping the country. It started as a way to help business executives successfully manage their personal lives while balancing all of the demands involved in leading large organizations. Now coaching is available for teenagers who want to achieve success and balance, gain confidence, prioritize goals, sharpen decision-making skills and improve relationships.

A therapeutic case management program for preschool-aged children who are survivors of physical, emotional and sexual abuse, and/or have aggressive and anti-social behaviors. ReConnect Peer Support Centers provide a safe and supportive environment where individuals gain confidence, build social skills, develop new friendships and engage in learning activities with peers. As a peer-operated program, members are involved in all aspects of ReConnect. Activities include: educational classes promoting independence, support groups, meals and dining out, arts and crafts, field trips, sporting events, shopping excursions, music, parties and dancing. TEAM Recovery (Treatment Empowerment And Meaningful Recovery) Ages: 18 and older

A grant-funded program through SAMHSA's Centers for Substance Abuse Treatment (CSAT) and Mental Health Services (CMHS), TEAM Recovery provides treatment for adults with mental illness and addiction who are involved in the criminal court system. The program coordinates collaborations with the 12th Judicial District Circuit and Drug Courts and community providers to provide treatment for mental illness, substance abuse and co-occurring disorders.

Mobile therapy assists clients who have difficulty getting to their initial intakes or follow-up appointments at one of our outpatient clinic locations. Mobile Therapy provides a solution so families do not go without necessary mental health services by bringing the service directly to the patient. Mobile Therapists meet clients in their homes or other locations for their appointments so that a transportation issue, time conflict, distance from a clinic or other barrier will not keep them from getting the help they need. Intensive In-Home Treatment Services Ages: Families with children ages 3-18
State: TN
Intensive In-Home Treatment provides high-intensity (10-12 sessions/month), time-limited (typically 90 days or more) therapeutic services for children and adolescents at imminent risk of out-of-home placement (acute psychiatric hospitalization, residential treatment, DCS custody) due to serious emotional illness which cannot be successfully treated in less intensive settings. Services are provided in the home and community and focus on short-term stabilization, with the ultimate goal being long-term stability for the member and family. In-home specialists are available 24/7 to respond to crises. In addition to the individual and family counseling component of the program, other services that may be provided include parenting training, behavior management, communication and relationship-building, self-management of moods and behavior, and school interventions.
Eligibility: 

Children and adolescents with significant behavior or emotional problems where recovery has been unsuccessful in outpatient treatment. Referrals may be made by a mental health center, the juvenile court, the Department of Children's Services or a self-referral.

Supportive Services for Veterans Families (SSVF) Ages: 18 and older (and family members of all ages)

Homelessness is a common struggle in America – with a growing number of families experiencing periods of homelessness each year. Centerstone offers Supportive Services for Veteran Families to provide assistance to Veterans and their families who are at risk for homelessness in Tennessee.

The SSVF program prevents homelessness by helping qualifying Veterans and their families gain access to the resources they need in order to successfully transition to or maintain permanent housing. Visit our website ssvfonline.org for more information.

Through the SSVF program, Centerstone’s Community Support Specialists provide guidance and coordination to Veterans and their families, helping them make informed decisions, understand their options and gain access to VA benefits. In addition, Community Support Specialists connect Veterans and families with other community resources including:

  • Health care services
  • Personal financial planning services
  • Transportation services
  • Income support services
  • Legal services
  • Child care
  • Daily living services

Temporary financial assistance payments may also be available to help with expenses such as rent, utility fees, security deposits, moving costs, transportation and child care.

What to Expect
Interested Veterans and their families work with a Centerstone Community Support Specialist to complete an eligibility screening and assessment. Once enrolled, participants in the SSVF program work closely with Centerstone Staff to identify barriers and determine what support services are necessary to meet essential needs.

Cost of Service
A grant-funded program through the U.S. Department of Veterans Affairs, SSVF is available at no cost to eligible candidates in 14 Middle Tennessee counties.

Eligibility: 

The program is offered to single Veterans or Veteran families where the head of the household or spouse is a Veteran.

In addition, eligible participants must have a household income that does not exceed 50 percent of the median area income.

School-Based Counseling and Case Management Ages: 17 and younger

School-Based Therapists/Case Managers work within preschool, elementary, middle and high school classrooms, helping students overcome behavioral, emotional or social problems that interfere with success at school and at home. Common issues may include: aggression, anxiety, depression, isolation, poor social skills, stress, trauma and truancy.

Integrated as members of the school team, Therapists/Case Managers serve youth in a familiar setting, offering little interruption to their school day. School-Based Services include: individual and group therapy, case management, family counseling, classroom observations, risk assessments, specialized training and support services for parents and teachers, collaboration with other community providers, and linkage to additional community resources.
