For youth who have emotional and behavioral problems that prevent them from functioning in a regular school setting. Public school coordinators refer students to Weems Academy where they receive counseling and therapy while maintaining their academic studies, in an effort to return the youth to their community school.

Substance Abuse Treatment with Co-Occurring Mental Illness Ages: 13 and older

Building Strong Families Ages: 17 and younger
State: TN
Building Strong Families is a grant funded program for children, birth to age 17, who are at imminent risk (within 24 hours) of being placed outside the home because of their parent's or caretaker's drug use. This program provides intensive family preservation services using the HOMEBUILDERS evidence-based model. The program provides crisis intervention, intensive in-home therapy for all members of the family, parenting and life skills training and referral to other needed services, such as substance abuse treatment. It is designed to keep children and families together.

Intervention Services for Addiction Disorders Ages: All ages

Working with family, friends or an employer, an Interventionist uses a team approach to confront an individual about the impact of their addiction. Through the intervention, the individual is encouraged to seek treatment. If the person agrees to treatment, the Interventionist provides the individual with professional and confidential advice in addressing addiction-related issues, assistance with entering the most appropriate treatment program, as well as transportation to treatment and case coordination during and following completion of treatment. Substance Abuse Treatment with Emphasis on Methamphetamine Ages: 18 and older

Substance Abuse Treatment for participating county residents consists of an intensive 16-week outpatient treatment program for adults who are abusing methamphetamine, alcohol, cocaine, other stimulants and emerging drugs.

Project SELF Ages: 12-18
State: TN
Project SELF (Students Experiencing Life Free) is a research-based substance use disorder treatment program for youth. The grant funded outpatient treatment program lasts six months and consists of 12-14 weekly individual sessions in the home. Case management continuing care is also provided to assess individual need and offer links to other helpful resource services within the community as well as healthy activity options. Participants will be tested to maintain substance abstinence during treatment.

For over 20 years, Centerstone’s innovative Therapeutic Preschool program has provided comprehensive evaluation and intensive treatment for children, ages 30 months to five years-old. This program is appropriate for children who have been victims of or are at high-risk of: neglect; emotional, physical or sexual abuse; have witnessed traumatic events; and/or display atypical emotions and behaviors. Early Childhood Network Ages: Birth to third grade and their families

Transitional residential services, assisting individuals in transition from psychiatric hospitalization or an unstable living situation to stable and more independent living situations.
Eligibility: An adult who is in need of transitional residential supervision. This can either be utilized as a step-down transition from the hospital to community living or as a diversion.

Co-Occurring Disorders Treatment Ages: 18 and older
State: TN
Co-Occurring Disorders Treatment is for individuals who have both a substance use disorder and a mental illness. This grant-funded program provides therapy, intensive outpatient counseling, case management and psychiatric services for both disorders, rapid HIV testing for individuals and their partners, limited assistance with transportation to treatment and referral to primary care when needed. Individuals with co-occurring disorders have a better chance of recovery from both disorders when they receive services that combine mental health and substance abuse treatment.

ReConnect Peer Support Centers provide a safe and supportive environment where individuals gain confidence, build social skills, develop new friendships and engage in learning activities with peers. As a peer-operated program, members are involved in all aspects of ReConnect. Project MAP Ages: 10-18

Project MAP (Methamphetamine Awareness and Prevention) is a grant funded program to prevent, reduce or delay the use and/or spread of meth abuse in high risk, rural Tennessee. The school-based prevention program serves middle and high school aged youth and is funded by the Substance Abuse and Mental Health Services Administration (SAMHSA). Project MAP recently expanded its outreach to include serving adults and youth through community presentations, service learning projects and other activities to increase community awareness and education.

Child and Youth Residential Treatment Services - Youth Group Homes Ages: 11-18
State: TN
These are highly structured programs addressing problems or behaviors keeping youth from being successful in the community. Residency lasts two to three months. While living in the home, youth attend public schools, participate in community recreation and learn independent living skills such as food preparation, banking, laundry and shopping.
The Lodge is a 6-bed residential facility (Level II) for girls ages 11 to 18 in Montgomery County. The average stay is 1 to 3 months. Services focus on rapid stabilization and quick transition back to home and community. Individuals are also linked to mental health treatment and other supports to help prevent the occurrence of a future crisis.

Case Management Services for Children and Youth Ages: 17 and under

Case Management Services is a voluntary in-home intervention and advocacy service. It is designed, whenever possible, to keep children and families together. The goal is to teach the child and families the skills they need to be stronger and healthier, and to be able to advocate for the child's needs. Case Managers link, refer, advocate and skill-build for families in the communities where they live.
Eligibility: Must be under 18 with a mental health diagnosis or a severe emotional disturbance. Has a history of out of home placements or difficulty in the home environment. Has difficulty adjusting to their natural environment - school, home, and/or community.

Not Alone, a company of Centerstone, is dedicated to empowering warriors and families by providing free, confidential support, program and resources that help them face the challenges of life after war. Such services provided include online support groups and in-person mental health services to soldiers returning from Afghanistan and Iraq. Through NotAlone.com, those in need have access to a 24-hour help line, free therapy services, online seminars and friendship with those who are facing similar challenges. Mobile Therapy Ages: 5 and older

Mobile therapy assists clients who have difficulty getting to their initial intakes or follow-up appointments at one of our outpatient clinic locations. Mobile Therapy provides a solution so families do not go without necessary mental health services by bringing the service directly to the patient. Mobile Therapists meet clients in their homes or other locations for their appointments so that a transportation issue, time conflict, distance from a clinic or other barrier will not keep them from getting the help they need. Staff members travel to the child and their family to complete the intake and refer them to appropriate care, whether that be Outpatient Counseling, Psychiatric Evaluation, Intensive In-Home Treatment Services, School-Based Services, Case Management, CTT or, in some instances, continued Mobile Therapy.

Intensive In-Home Treatment Services Ages: Families with children ages 3-17
State: TN
Intensive In-Home Treatment provides high-intensity (avg. 10 hours/month), time-limited (typically 90 days or more) therapeutic services for children and adolescents at imminent risk of out-of-home placement (acute psychiatric hospitalization, residential treatment, DCS custody) due to serious emotional illness which cannot be successfully treated in less intensive settings. Services are provided in the home and community and focus on short-term stabilization, with the ultimate goal being long-term stability for the member and family. In-home specialists are available 24/7 to respond to crises. In addition to the individual and family counseling component of the program, other services that may be provided include parenting training, behavior management, communication and relationship-building, self-management of moods and behavior, and school interventions.
Eligibility: 

Children and adolescents with significant behavior or emotional problems where recovery has been unsuccessful in outpatient treatment. Referrals may be made by a mental health center, the juvenile court, the Department of Children's Services or a self-referral.

The SSVF program prevents homelessness in Middle Tennessee by helping qualifying veterans and their families gain access to the resources they need in order to successfully transition to or maintain permanent housing. This program is for adolescents who have a chemical dependency problem that requires more frequent and intensive therapy, or those with a chemical dependency problem coupled with emotional or behavioral problems. Centerstone staff members provide individualized treatment plans that address substance abuse and mental health needs.
